Degree Completion

If you are approaching the end of your degree program, there are several forms you need to complete in order to receive your degree, and some important guidelines, instructions, and checklists you should refer to as you prepare your dissertation or thesis for submission. You may download all of the necessary information and forms from the Graduate School website, or if you prefer, you can you see the student records coordinator in the Graduate School, who will give you a graduation packet that contains a memorandum explaining the necessary steps for graduation and all of the forms you will need to complete.

Degree Completion Procedures for PhD Candidates
  1. Complete the Application for Candidacy and return it to the Graduate School office. PhD candidates must be admitted to candidacy at least one semester before applying for the degree.
  2. You must be registered during the semester in which your degree will be awarded. The residency course 999GSAS satisfies this requirement.
  3. Complete the Application for Degree and return it to the Graduate School office. The Graduate School assesses a $25 late fee for applications submitted after the deadline. If you apply and do not complete all requirements, you must apply again and register for the semester in which you will receive the degree.
  4. Complete the Report of Completion of Requirements for Doctoral Degree (sometimes referred to as the Degree Clearance Form) and submit it to the Graduate School office. This form may be submitted before submitting the dissertation or along with the dissertation. Deadlines for submission of this form are listed in The Graduate School Handbook and on the Graduate School website. Candidates must resolve all incomplete grades, administrative F, and P grades before submitting the Degree Clearance Form.
  5. All PhD candidates must complete the Survey of Earned Doctorates Form and submit it along with the dissertation to the graduate school.
  6. Complete and submit the University Microfilms Agreement Form.
  7. You must submit five copies of your dissertation to the dean of the graduate school by the deadline stipulated for the semester in which you plan to graduate (see the academic calendar on the Graduate School website or in the Graduate School handbook). Also, please review the Submitting Your Dissertation instructions, included below.

Submitting Your Dissertation:
Well in advance of the deadline for submitting the dissertation, you should carefully read the Instructions for the Preparation of Theses and Dissertations. The Graduate School will not accept dissertations that do not adhere to the guidelines listed in these instructions.

Number of Copies: You must submit an original and four copies of your dissertation to the Graduate School office by the specified deadline. For these dates, see the Graduate School academic calendar on this website. The original and three copies must be on 20# or 24# acid-free paper. The original and one copy go to the university library: the original for the library archives; the copy for circulation. Two copies are sent to your department, one for the department to keep and one for you. The fifth copy will be used for University Microfilms Inc. and is considered disposable; therefore, you may print this copy on regular or disposable copy paper. Be sure the circulation agreement is signed on all copies of your dissertation.

Binding Fee: All students submitting dissertations must pay an $85.00 binding fee. You must submit this fee at the cashier window in the Boisfeuillet Jones Building. Keep your receipt; you will need to submit it to the Graduate School office along with your dissertation. The binding fee covers the cost of binding four copies of your dissertation: the original for the university library’s archives and one copy for library circulation; one copy for you; and one for your department. You may arrange for additional copies to be bound by contacting the National Library Binder Company, 100 Hembree Park Dirve, Roswell, GA 30076, 770-442-5490. Previously bound or stapled dissertations will not be accepted.

  3. You must submit your thesis to the dean of the graduate school by the deadline stipulated for the semester in which you plan to graduate. Please review the Submitting Your Thesis instructions, included below.

Submitting Your Thesis:

Number of Copies: If you choose to copyright your thesis, you must turn in the original and four copies. The original and three copies must be on acid-free 20# or 24# paper. The additional copy, designated as the University Microfilms copy, can be on disposable paper.

If you do not choose to copyright your thesis, you must turn in the original and three copies on acid free 20# or 24# weight paper.

Binding Fee: All students submitting theses must submit a $45.00 binding fee. You must submit this fee at the cashier's window in the Boisfeuillet Jones Building. Keep your receipt; you will need to submit it to the Graduate School office along with your thesis. The binding fee covers the cost of binding four copies of your thesis: the original for the university library’s archives and one copy for library circulation; one copy for you; and one for your department. Option to Copyright Theses
You have the option to copyright your thesis.If you plan to copyright, you must submit the following in addition to the requirements listed above:

University Microfilms Agreement Form and Copyrighting:
All PhD candidates must complete the University Microfilms Agreement Form and attach a copy of the abstract and the abstract cover page to this form.

Although submitting the UMI form is mandatory for all PhD candidates, the decision to copyright your dissertation or thesis is optional. If you decide to copyright the dissertation, you can either do it yourself, through the U.S. Copyright Office, or you can have UMI do it for you.  The former is likely to be less expensive, and not difficult.  If you want UMI to do it, please include a check or money order in the amount of $65 payable to ProQuest Information and Learning.  For more information on copyright issues, please visit the Electronic Theses and Dissertations Repository page on intellectual property.
