IACUC Forms

Options for obtaining IACUC forms:

The current version of the IACUC application is 3/18/08. All other forms are currently versioned 11/12/07 or later. You must submit the newest version of these forms for all new and renewal applications.

You can obtain MS Word Windows versions directly from the IACUC office. You can request them via phone 404.712.0734 or e-mail IACUC@emory.edu. They will be sent to you as an e-mail attachment.

You can obtain the MS Word Windows version via download from this page.

Instructions for Downloading the forms from this page:

Click on the link for the document you wish to download.
MS Internet Explorer will automatically open the document. You must then save it to a location on your harddrive.
With Netscape, a "Save As..." dialog box will appear. Enter the appropriate location to store the file and the file name for the download file.
The file will take a few moments to download.
To view the file you downloaded, you will need to open the application (e.g. MS Word).

You must email the completed form to IACUC as a "Word Document" and fax or send a hardcopy of the face page and COI page.

IACUC Protocol Modification form with Guidelines included - revised 2/26/08
Please email the completed form to IACUC.  The PIs signature is not required.

IACUC Credentials form for Investigators, Trainees and Technicians - revised 2/26/08
(every person involved with the Protocol MUST complete a copy of this form to be kept on file in the IACUC office). Submit electronics copies of the completed form to IACUC@emory.edu.
