The IACUC office will log the incident into the database
established for tracking such incidents and inform the Chair of the IACUC,
or the Vice Chair if the Chair is unavailable. Each step of the following
procedure will be copied to the IACUC office where it will be recorded in
this database.

The Chair, or Vice Chair, will immediately inform the Associate
Dean for the School of Medicine. Together they will prepare an initial report
to notify the appropriate authorities (USDA and OPRR) of the incident by
phone.

A subcommittee, composed of three IACUC committee members
will be appointed, within 24 hrs, by the Chair or Vice Chair, to investigate
the incident.

The subcommittee will meet with the parties involved and
the Facility Supervisor, and the Veterenarian responsible for oversight
of the Facility involved.

The subcommittee will prepare a draft report describing
the incident and making recommendations to resolve the incident and prevent
its recurrence. The report will be sent to the Chair, or Vice Chair, and
the Associate Dean.

This report will also be sent to the IACUC members and
it will be discussed at the next regular IACUC meeting.

The report, and any additional recomendations from the
IACUC committee, will be sent to the Facility involved. The veterenarian
responsible for the facility will issue a report documenting the actions
they have implemented. This report will be sent to Chair, or Vice Chair,
and the Associate Dean.

The Chair, or Vice Chair, and the Associate Dean will prepare
a summary report to be sent to the appropriate regulatory agencies over
both signatures. This entire process should take no more than one month
to accomplish depending upon the time the incident occurs relative to the
next IACUC committee meeting.
