All submissions must include the following:
- A completed IACUC Application;

- The PIs and the department
chairperson's signature are required on the face page of the application
and the PIs signature on the conflict of interest (COI) page. These documents are not
required for review, but are required before final approval will be granted.
The face page and COI page must be mailed,
faxed, or scanned and emailed to the IACUC office prior to final approval. ;

- A copy of your grant
narrative (sections A-F only of NIH grants) or a research proposal in
University Research Committee format; ALSO, if this is a 3 year renewal, the IACUC
requires a progress report be submitted summarizing the work accomplished in the last
three years. This should include an accounting of the animals used and a description of any
problems that occurred with procedures, surgery, etc. A copy of the PHS progress report form
(PHS 2590) will suffice for this progress report.

- If any animals are listed in
Pain Class D (Painful/Stressful WITH Analgesia, Anesthesia, Tranquilizers)
or E (Painful/Stressful WITHOUT Pain or Stress Relieving Measures), the
Principal Investigator is required by law to document that alternatives to
procedures that may cause pain or distress to animals have been
considered. A computer-assisted literature search is considered by the
USDA to be the best method to check for non-painful, non-stressful
alternatives. Therefore, you are required to describe the date and the
database(s) searched (keep copies of the search results in your files).

- A copy of a veterinary
consult and your response. Contact the Division of Animal resources
(vet_cons@dar.emory.edu, 404-727-3248) or Yerkes National Primate Research Center
(vet_cons@rmy.emory.edu, 404-727-7001) as appropriate, to arrange for the consultation.
If animals are to be used at both Emory and Yerkes, then a consultation should be
obtained from each.; For protocols involving non-human primates,
a written enrichment consult is also required before
submission of your application to the IACUC. The IACUC form must be
submitted for all consultations on or before the last
Wednesday of the month. You will be provided with a written
list of concerns or potential problems. Changes in response to
all concerns must be incorporated in the
final draft of the application that is submitted to the IACUC on or before
the first Wednesday of the month. Adequate attention to the consultation(s) greatly
improves the probability that your application will be quickly approved.
All consultation forms will be forwarded to
the committee to help in their deliberations.
Submissions will not be reviewed until all of the above requirements
are fulfilled.
