It has long been recognized that strictly relying on the classical case definition has led to delay in treatment of children who do not meet the classic Kawasaki syndrome criteria, yet still develop coronary artery aneurysms (CAA). Recently, there have been revisions made to guidelines that should assist in identifying patients at risk of CAA who do not meet the classic criteria. Since these revisions have not gone through prospective testing, it is recommended that these guidelines undergo analysis to determine their sensitivity and specificity for preventing CAA. Additionally, given that neither the agent(s) nor the pathophysiology that result in pediatric CAA are known, the illness should be properly labeled as Kawasaki syndrome instead of Kawasaki disease. This distinction will have positive ramifications for diagnosis and research. The adoption of this nomenclature will assist in the identification of those children at risk for CAA.
